A long-form Dallas birth certificate is an authorized government-issued copy of the original document. It includes any corrections made to the birth record over time. It carries the registrar's raised seal, the registrar's signature, and the date the certificate was filed. Long-form certificates are required for passports, driver's licenses, and most legal purposes.

Who Can Get a Mesquite Birth Certificate
