Texas Birth Certificate Types
Texas issues a few different birth certificate types. The Cochran County Clerk and the state DSHS both provide all types. Knowing which one you need will save time.
The long-form birth certificate is a full certified copy of the original record. It shows the complete history of corrections and includes parent names, place of birth, attending physician, and the registrar's embossed seal and signature. This form is required for passports, driver's license in most states, and citizenship applications. It is the most complete and widely accepted type.
The short-form certificate is a certified abstract showing only current information. It lists the name, date of birth, place of birth, sex, and parent names. No correction history appears. Short-form certificates are valid for school, employment, insurance, and most domestic legal uses but are not accepted for passport applications.
An heirloom birth certificate is a decorative document with no legal value. It cannot be used as proof of identity or eligibility for any benefit or official purpose.
