Texas Birth Certificate Types
Texas issues three main types of birth certificates. The Comanche County Clerk and the DSHS both provide all types.
The long-form birth certificate is a full certified copy of the original document. It includes parent names, place of birth, attending physician, and a history of all corrections made to the record. The registrar's embossed seal and signature appear on this document. Required for passports, driver's licenses in most states, and citizenship applications.
The short-form certificate is a certified abstract showing current data only: name, date of birth, place of birth, sex, and parent names. Accepted for school, employment, insurance, and most domestic uses. Not accepted for passport applications.
The heirloom certificate is decorative only. It has no legal validity and cannot be used for any official purpose.
