Birth Registration in Uvalde County
All births in Texas must be registered within five days under Texas Health and Safety Code Chapter 192. The doctor, midwife, or hospital administrator attending the birth files the certificate with the local registrar. For births in Uvalde County outside city limits, the registrar is the County Clerk. For births inside Uvalde city limits, the City also serves as a local registrar.
Once filed at the local level, a copy goes to the Texas DSHS office in Austin. That means records exist both at the county or city level and at the state level. You can request a certified copy from either. However, for very old records or cases where a birth was missed at the state level, the local office is the best place to start.
If a birth in Uvalde County was not registered at the time, Texas allows delayed birth certificate filings. The DSHS handles these. Visit dshs.texas.gov for the requirements and forms needed to file a delayed certificate.

The long form is a copy of the original record and is used for passports, name changes, and legal proceedings. Standard items on a Texas birth certificate include:
- Child's full legal name at birth

- Full names of both parents
- Mother's maiden name
- Attending physician or midwife's name
